As a Clinical Liaison (LPN/RN) at Aspire Senior Living, you will play a critical role in providing exceptional care and support to our residents. We are seeking a compassionate and experienced nurse to join our team and help us deliver high-quality personal care services in a warm and welcoming atmosphere.

About the Role

As a Clinical Liaison (LPN/RN), you will serve as a liaison between our healthcare team and residents in our Southeast communities, ensuring that their needs are met and exceeded. Your responsibilities will include:

• Assessing and developing individualized care plans for residents

• Providing direct care and support to residents with varying levels of need

• Collaborating with physicians and other healthcare professionals to ensure coordination of care

• Communicating effectively with residents, families, and staff to ensure seamless transitions and resolutions

• Participating in quality improvement initiatives to enhance the overall resident experience

Requirements

• Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensure in the state of Missouri

• Experience as a Clinical Liaison or related field setting preferred

• Ability to communicate effectively with residents, families, and staff in our Southeast communities
