Admission Coordinator

We are looking for an individual to join our mission of providing exceptional health care services through innovation, continuing education and evidence-based practice, while recognizing and encouraging the contributions of our employee, as we strive to meet the specialized health care needs of our communities.Admission Coordinator - Non-ClinicalAdmission Role would include:Review all referrals submitted by hospitals for admission to rehab and/or LTCOpen communication with hospital discharge planners on acceptance or denial of patient referralsEnsure required referral information and data are obtained timely prior to an admission arrivingEntering patients in the EMRFollow up daily on all pending referralsCompletion admission paperworkBridge communication with the team regarding new admissionsBasic understanding of insurance, electronic medical records and medical terminology preferredGeneral use of Microsoft Office Tools Qualifications:Must have a genuine interest in geriatrics and assuring quality medical records of residents. Ability to adapt to frequent changes in routine.Experience: Minimum of three years in the medical field.  Prefer LPN, RN, MSW, but college courses would be acceptable.Education: High school graduate or equivalent.Physical Requirements: Must be able to lift up to 50 lbs. Ability to walk, climb stairs, push and/or pull equipment and residents as needed. Prolonged standing or walking. Able to maintain high energy level throughout an eight hour shift.
