As a PT Social Worker, you’ll partner closely with the SW/CM to drive strong resident outcomes by supporting each person’s physical, emotional, and psychosocial well-being. This role exists to keep transitions smooth, communication clear, and resident rights front and center—every single day.

Key responsibilities include assisting with discharge planning, coordinating communication between residents, families/representatives, and the interdisciplinary team, and advocating for resident rights in line with Fellowship standards.

Required qualifications/skills:
- Bachelor’s degree in Social Work
- Minimum two years’ experience in geriatric care
- Excellent interpersonal/public relations skills; strong organization
- Ability to counsel individuals in crisis; knowledge of senior community resources
- Intermediate computer skills; working knowledge of the MDS
- Understanding of Medicare/MA/managed care/private pay basics; medical terminology

As the day builds, you balance focused conversations with timely follow-ups, keeping details organized and documentation current. You’ll shift gears between office work and on-unit presence, staying visible, calm, and ready when emotions run high.

By day’s end, you’ve helped keep plans on track, questions answered, and the team moving forward with confidence—then you wrap up knowing your 25 hours each week truly count.
