Summary: A home health agency scheduler coordinates patient care, manages staff schedules, ensures compliance with care plans and payer requirements, and serves as the central communication link between patients, caregivers, and the agency.

Core Responsibilities
 

Scheduling and Coordination: The scheduler manages daily schedules for home health staff, including nurses, therapists, and aides, based on the patient’s plan of care (POC) and payer authorizations. They ensure visits are scheduled according to patient needs, service frequency, and location, while accommodating emergencies or changes in care requirements 
 

Patient and Caregiver Communication: Schedulers act as the liaison between patients, families, and caregivers, confirm appointments, handling cancellations, and addressing scheduling conflicts. They ensure that patients receive timely care and that caregivers are matched appropriately based on skills and availability

Compliance and Documentation: They track and document missed visits, notify physicians of changes affecting the POC, and ensure adherence to CMS and payer guidelines. This includes managing formal and pending authorizations to maintain reimbursement compliance. 

Administrative Duties: Schedulers maintain accurate scheduling systems, update team managers on conflicts, and coordinate with other healthcare professionals to integrate all required services into patient care plans. They may also assist with client intake and case management. 
They proactively resolve scheduling conflicts, optimize staff utilization, and implement best practices for payer verification, authorizations, and communication to streamline patient care and agency operations. Processes intake telephone calls. 

 
Skills and Qualifications 
Organizational Skills: Ability to manage multiple schedules and prioritize tasks effectively. 

Interpersonal Skills: Strong communication to interact with patients, caregivers, and healthcare professionals. 

Technical Skills: Proficiency with electronic health records (EHR) and scheduling software. 

Problem-Solving: Capacity to handle unexpected changes, emergencies, and staffing shortages. 

Attention to Detail: Ensuring accurate documentation and compliance with care plans and payer requirements.

Physical Requirements: Able to see/hear/speak sufficiently to participate in written/verbal communication. Able to deal effectively with stress. Able to participate in light physical activity, including data entry, light lifting and extending period of sitting. 

 Education:  
High School Diploma 

 Experience:/Skills Required:  
1-2 years of Home Health care experience and scheduling. 

Work Environment: In Office 

 Pay rate: $20-$22 an hour
