The Lead Non-Clinical Case Manager provides comprehensive non-clinical case management and care coordination services in support of the Vermont Army National Guard (VTARNG). The position supports service member wellness, operational readiness, and deployability by coordinating resources and services across VTARNG facilities and external organizations.Duty LocationGreen Mountain Armory, Colchester, VTWork ScheduleMonday through FridayTypical duty hours between 0730 and 1630Support for weekend Soldier Readiness Events (SRP/PHA) and mobilizations as requiredPrimary Duties and ResponsibilitiesCoordinate all aspects of the care of individual Service MembersEnsure proper utilization of services and resourcesEstablish and maintain an eCase profile for Soldiers referred by a providerTrack Soldiers' care from identification through resolutionMaintain HIPAA standards and guidelinesDocument care in eCase, MEDCHART, MEDPROS, eProfile, MHA/PHA, AHLTA, AVS, MODS, and MWDEPrepare Soldier records for Medical Evaluation Boards (MEB/PEB)Coordinate treatment plans with appropriate medical facilitiesMaintain Soldier medical records in accordance with AR 40-66Produce monthly case status and profile reportsUpload medical documents to Health Readiness Record (HRR)Additional ResponsibilitiesAttend meetings as neededMaintain required records and documentationSupport administrative and coordination tasksAssist with process improvement and continuity of operationsMinimum QualificationsRegistered Nurse (RN) with an active, unrestricted active Vermont state license that is in good standingExperience in non-clinical case management or care coordinationAbility to coordinate services across multiple organizationsStrong organizational, communication, and interpersonal skillsAbility to work effectively with military service members and Government personnelProficiency in written and spoken EnglishAbility to obtain a Common Access Card (CAC)Maintain HIPAA and PII compliance at all times
