RN- UAS Homecare (Hybrid) Albany

Join our team at WCS Healthcare Partners as a Clinical Assessment RN, where you will play a crucial role in evaluating and coordinating care for consumers within Albany County. Responsibilities

• Perform initial in-home pre-evaluation assessments of consumers.
• Review consumer medical documentation and health referral forms.
• Enter evaluation data into the UAS-NY online application.
• Code UAS assessments correctly based on guidelines.
• Document concerns, conflicting information, and outcomes.
• Emphasize continuity of care to reduce fragmentation.
• Discuss health care options and service vendor/waiver options with consumers.
• Initiate enrollment into managed care health plans.
• Stay updated on regulatory changes related to assessments.
• Improve care coordination with interdisciplinary team members.
• Review non-qualified assessments with the Quality Assurance Department.
• Collect quality review data and document case notes.
• Identify health promotion and illness prevention opportunities.
• Maintain knowledge of community resources and network services.
• Meet performance criteria and perform assigned duties.

Requirements

• NYS RN Licensure required.
• 2 years of Homecare experience required.
• Must be able to travel throughout Albany County.
