Physical Therapist Job Description

Provide physical therapy services within the legal scope of practice for the state, adhering to all licensure requirements.

  • Maintain valid licensure and certifications as required by state and federal regulations.
  • Conduct initial evaluations, progress notes, 30-day reassessments, and 60-day recertifications in accordance with Medicare, state, and organizational standards.
  • Develop, implement, and update individualized plans of care aligned with physician orders and patient needs.
  • Deliver skilled interventions and maintenance therapy programs as permitted by defined scope of practice.
  • Directly supervise Physical Therapist Assistants (PTAs) and Home Health Aides in compliance with state practice acts.
  • Consult with physicians and interdisciplinary team members regarding treatment adjustments and patient progress.
  • Educate patients and caregivers on home exercise programs, mobility, and adaptive strategies to promote independence and safety.
  • Prepare and submit OASIS assessments, recertifications, transfers, resumptions of care, and discharges within defined timeframes.
  • Appropriately apply ICD-10 coding for accurate and compliant documentation.
  • Present in-service programs periodically to staff and contribute to educational initiatives.
  • Actively engage in Quality Assessment and Performance Improvement (QAPI) activities to support continuous compliance and clinical excellence.
Other Duties
  • Provide care and services in accordance with infection control, safety, and risk prevention protocols.
  • Accurately document clinical interventions, care coordination, and communication in a timely manner.
  • Participate in interdisciplinary meetings, case reviews, and continuing education opportunities.
  • Maintain compliance with HIPAA and all privacy and confidentiality standards.
  • Represent the organization positively and professionally in patients’ homes and community settings.
  • Work independently with minimal daily oversight, demonstrating initiative in completing responsibilities.
  • Provide courteous, timely, and professional customer service to patients, families, and partners.
  • Exhibit adaptability to changing responsibilities and flexibility in completing assignments.
  • Demonstrate consistent and effective use of organizational technology systems and tools required for the role, including communication, documentation, productivity, and data management platforms.
  • Adhere to all organizational policies, including job descriptions, mission, and employee guidelines.
  • Communicate and interact professionally and respectfully with others to support team goals.
  • Perform other duties as assigned to effectively discharge the responsibilities of the position and support the best interests of the organization.
Qualifications
  • Graduate of an accredited physical therapist curriculum approved by relevant accreditation bodies.
  • Currently licensed as a Physical Therapist in the state of practice.
  • Two years of clinical experience preferred; home health experience is a plus but not required.
  • Ability to exercise sound clinical judgment and work independently in varied patient care settings.
  • Strong communication and interpersonal skills with sensitivity to diverse backgrounds.
  • Basic computer proficiency; willingness to learn EMR systems and OASIS documentation.
  • Current CPR certification.
  • Commitment to compliance with Medicare/Medicaid Conditions of Participation, state regulations, and agency policies.
  • Reliable transportation, valid driver’s license, and auto insurance required for home visits.
