🩺 Position Overview

We are seeking an experienced Physical Therapist (PT) to provide compassionate, patient-centered rehabilitation services in a Skilled Nursing Facility setting. The ideal candidate will help patients improve mobility, strength, functional independence, and overall quality of life through individualized treatment plans.

✅ Requirements

🎓 Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program
📋 Active California PT License required
💪 Minimum 3 years of Physical Therapy experience preferred
🏥 Skilled Nursing Facility or similar rehabilitation experience preferred
🧠 Strong knowledge of therapeutic exercises, modalities, and adaptive equipment
🤝 Excellent communication and interdisciplinary collaboration skills
❤️ Compassionate, patient-focused approach to care

📌 Responsibilities

✔️ Perform comprehensive physical therapy evaluations
✔️ Develop and implement individualized treatment plans
✔️ Provide therapeutic interventions to improve strength, mobility, and function
✔️ Monitor patient progress and modify treatment plans as needed
✔️ Educate patients and families on exercises, safe mobility, and home modifications
✔️ Collaborate with nurses, physicians, occupational therapists, and other members of the care team
✔️ Maintain accurate and timely clinical documentation
✔️ Ensure treatment plans comply with facility policies and regulatory standards
