Description
American Traveler is seeking a Physical Therapist for a Home Health assignment, requiring active state licensure and reliable transportation for community-based patient visits.
Details
- Home health setting providing physical therapy services to patients in their homes
- Patients seen within a 45-mile radius of the Albuquerque area, including Tijeras, Rio Rancho, Bernalillo, Bosque Farms, Los Lunas, and Belen
- Monday through Friday schedule
- 13-week assignment
Requirements
- Active PT license in the state of practice required (license required if applicable)
- Graduate of an accredited physical therapist program recognized by CAPTE, APTA, or CAHEA
- Current CPR certification required
- 2 years of clinical experience preferred; home health experience a plus but not required
- Reliable transportation, valid driver's license, and auto insurance required for home visits
- Basic computer proficiency with willingness to learn EMR and OASIS documentation systems
Additional Information
- Conduct initial evaluations, progress notes, 30-day reassessments, and 60-day recertifications per Medicare and agency standards
- Develop and implement individualized plans of care aligned with physician orders
- Supervise Physical Therapist Assistants and Home Health Aides in compliance with state practice acts
- Consult with physicians and interdisciplinary team members on treatment adjustments and patient progress
- Educate patients and caregivers on home exercise programs, mobility, and adaptive strategies
- Complete OASIS assessments, recertifications, transfers, resumptions of care, and discharges within agency timeframes
- Apply ICD-10 coding for accurate documentation
- Participate in QAPI activities and periodic staff in-service programs
- Work independently with minimal daily oversight in varied home care settings
- Maintain compliance with HIPAA and infection control, safety, and risk prevention protocols
