Nurse Liaison

• Visiting multiple area hospitals and completing on site assessments
• Meeting patients and families and helping them understand what to expect when they come
• to Pomeroy Living
• Reviewing clinical information to determine whether we can safely meet someone's needs
• Working closely with hospital case managers and discharge planners
• Making sure our communities have the clinical information they need before a resident arrives
• Following referrals and helping move appropriate admissions forward
• Building and maintaining relationships with our hospital partners
• Representing Pomeroy Living in hospital meetings and community events
• Occasionally supporting our Central Intake team when needed

• You are clinically sharp, organized, personable, and comfortable walking into a hospital and starting a conversation.
• You understand that this job is part nurse, part relationship builder, part problem solver, and occasionally part detective.
• A Michigan RN or LPN license is needed, along with experience in skilled nursing, post-acute care, or hospital liaison work, admissions, or referral management.
• Most importantly, you need good clinical judgment and the ability to connect with people. A family deciding where Mom or Dad goes after the hospital is making a big decision. We want someone who understands that.

Compensation details: 70000-90000 Yearly Salary
