As a Nurse Practitioner with Carease Health - IL, you will play a crucial role in delivering high-quality preventative care to residents in skilled nursing facilities. Our company prioritizes proactive measures for enhanced physical health and well-being, incorporating Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM) into our comprehensive preventative care program.

About the Role:

As a part-time Nurse Practitioner, you will provide high-quality preventative care to residents in skilled nursing facilities through on-site visits and remote work from home. This role is perfect for those who want to make a meaningful impact in the lives of patients while enjoying a flexible schedule.

Responsibilities:

• Provide high-quality preventative care to residents in skilled nursing facilities

• Conduct assessments, develop treatment plans, and prescribe medications as needed

• Collaborate with healthcare teams to ensure comprehensive care and minimize hospitalizations

• Utilize RPM, CCM, and CoCM to enhance patient outcomes

• Maintain accurate and timely documentation of patient care and outcomes

• Participate in quality improvement initiatives to enhance patient care standards

Requirements:

• Active NP license (in the state of practice)

• 1+ year of experience in Geriatric care and documentation review

• Excellent communication, collaboration, and teamwork skills

Additional Information:

• Salary: $23,000 - $25,000 ANNUALLY
