Key Responsibilities- Evaluate and treat patients in a skilled nursing facility setting
- Develop and implement individualized physical therapy treatment plans
- Monitor and document patient progress toward therapy goals
- Collaborate with interdisciplinary care team to optimize patient outcomes
- Maintain accurate and timely clinical documentation in accordance with facility standards
- Active Physical Therapist (PT) license in the state of Wisconsin
- Graduate of an accredited Physical Therapy program
- Ability to fulfill a 13-week contract commitment
- 35-hour weekly hour guarantee
- Prior experience working in a skilled nursing facility (SNF) setting
- Familiarity with Medicare documentation and compliance requirements
- Guaranteed 35 hours per week providing stable, consistent work throughout your contract
