Physical Therapist (PT)
LocationSanta Maria, CA
Schedule / Job TypeContract | Days | Monday–Friday, 8:00 AM–5:00 PM (flexible); every 3rd weekend required; minor holidays as scheduled
Key Responsibilities- Provide physical therapy services to patients in a home-based care setting within an assigned geographical region
- Evaluate and treat patients with diagnoses including orthopedic conditions, strokes, joint disorders, and spinal injuries
- Complete and maintain accurate clinical documentation using Cerner EMR
- Complete OASIS assessments as required by government mandate
- Collaborate with interdisciplinary team members including OT, Speech, and support staff
- Adhere to professional dress standards including business attire and white lab coat
- Active Physical Therapist (PT) license
- Current BLS certification
- Minimum of one year of experience as a Physical Therapist
- Demonstrated experience with OASIS documentation
- Ability to work every 3rd weekend and minor holidays as needed
- Prior experience in a home health or home-based care setting
- Familiarity with Cerner EMR platform
- Work within a structured and supportive interdisciplinary team environment serving patients across a defined geographic region
- Enjoy a primarily weekday daytime schedule with flexibility built into the role
