Physical Therapist (PT) – Home Health
LocationVernon, CT
Schedule / Job TypeContract | Full-Time | Days | Monday – Friday with weekend rotation (approximately every 6 weeks)
Key Responsibilities- Perform Start of Care evaluations for home health patients within an assigned territory
- Deliver skilled physical therapy services to patients in their homes within an approximate 40-mile radius
- Meet productivity expectations of 28–30 points per week
- Document all patient care activities accurately using HCHB electronic medical records
- Oversee and direct the work of Physical Therapist Assistants (PTAs) as applicable
- Participate in weekend rotation coverage as scheduled
- Active Physical Therapist (PT) license in the state of Connecticut
- Prior home health experience required
- Experience with HCHB (Homecare Homebase) EMR system required
- OASIS experience required
- Ability to travel within an approximate 40-mile radius of the assigned territory
- Experience supervising or directing Physical Therapist Assistants (PTAs)
- Strong organizational and time management skills to support productivity goals in a home health setting
- Mileage reimbursement provided at the federal rate, and a company tablet is issued for documentation
- Guaranteed 40-hour work week with a structured Monday–Friday daytime schedule and no on-call requirement
