Physical Therapist (PT) – Home Health | Travel Contract

💼 30 Guaranteed Hours/Week
Position Overview: Minimum 3 years in Physical Therapist (PT) – Home Health
Provide skilled physical therapy services to patients in their homes
Conduct initial evaluations, reassessments, progress notes, and recertifications
Develop and implement individualized plans of care
Provide skilled interventions and home exercise programs
Educate patients and caregivers on mobility, safety, and adaptive strategies
Coordinate care with physicians and interdisciplinary teams
Complete OASIS documentation and maintain accurate clinical records
Supervise PTAs and Home Health Aides as required
Participate in QAPI, case reviews, and continuing education
Requirements: Minium
Graduate of an accredited Physical Therapy program
Active PT license in New Mexico
Current CPR certification
Two years of clinical experience preferred
Home Health experience is a plus but not required
Strong clinical judgment and ability to work independently
Reliable transportation, valid driver's license, and auto insurance required
Comfortable traveling to patients' homes within the assigned service area
