Key Responsibilities- Evaluate and treat patients in a skilled nursing facility setting
- Develop and implement individualized physical therapy treatment plans
- Monitor and document patient progress toward established goals
- Collaborate with interdisciplinary care team to ensure optimal patient outcomes
- Maintain compliance with all facility and regulatory requirements
- Active Physical Therapist (PT) license in the state of Illinois
- Graduate of an accredited Physical Therapy program
- Current CPR/BLS certification
- Previous experience working in a skilled nursing facility (SNF) environment
- Familiarity with Medicare documentation and compliance requirements
- Opportunity to make a meaningful impact on patient recovery and quality of life in a supportive SNF setting
- Gain valuable contract experience with a defined 13-week placement and a day-shift schedule
