Key Responsibilities- Evaluate and treat patients with speech, language, voice, and swallowing disorders in a skilled nursing facility setting
- Develop and implement individualized treatment plans based on patient evaluations and clinical goals
- Monitor and document patient progress, adjusting treatment plans as clinically appropriate
- Collaborate with interdisciplinary care teams to coordinate patient care and discharge planning
- Educate patients, families, and caregivers on therapeutic techniques and home programs
- Active Speech Language Pathologist license in the state of Colorado (or eligibility)
- Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from ASHA
- Experience working in a skilled nursing facility or similar post-acute care environment
- Strong clinical documentation skills
- Experience with dysphagia evaluation and treatment, including modified barium swallow studies
- Familiarity with Medicare/Medicaid documentation requirements in SNF settings
- Experience with PDPM billing and documentation
- Opportunity to work in a scenic Colorado community on a structured 13-week contract with a defined start date
