π₯ About the Assignment
This position is primarily based within the Skilled Nursing Facility (SNF)/Extended Care Center (ECC) but may flex into the hospital's inpatient rehabilitation department based on patient needs.
You'll work with a diverse patient population, including:
- Short-term rehabilitation patients
- Long-term care residents
- Post-acute recovery patients
- Inpatient rehabilitation patients
This role offers a unique opportunity to broaden your clinical experience while delivering high-quality, patient-centered care.
πΌ What You'll Be Doing
As a Physical Therapist, you will:
β Evaluate and treat rehabilitation patients in the SNF setting
β Develop individualized treatment plans
β Provide therapeutic interventions to long-term care residents
β Collaborate closely with Occupational Therapy and interdisciplinary team members
β Participate in discharge planning and patient care recommendations
β Attend weekly patient care conferences
β Support inpatient rehabilitation services as needed
β Help patients improve mobility, strength, function, and independence
