Job Opportunity Position: Speech and Language Pathologist (SLP) Specialty: Long Term Care (LTC) Shift Details
This position requires Monday to Friday shifts, with 8-hour workdays. Start times are flexible, provided they are communicated with the Director of Rehabilitation (DOR). Therapists may begin their shifts any time between 6 AM and 10 AM.
Weekend RequirementsWeekend work is not required unless a holiday occurs during the Monday to Friday workweek, in which case hours may be made up on the weekend.
Job Responsibilities- Screen, evaluate, and rehabilitate patients with communication, cognitive, or swallowing disorders, developing appropriate plans of care in compliance with all regulatory and clinical standards.
- Utilize standardized assessments and evidence-based practices to guide clinical interventions.
- Assess patient progress and adjust treatments accordingly within the clinical record to meet regulatory and practice requirements.
- Evaluate treatment outcomes for effectiveness and modify services based on patient evaluations, making referrals as necessary.
- Provide consultation and counseling support to patients, families, caregivers, and other service providers regarding speech disorders.
- Communicate effectively with supervisors and other health team members about patient progress, barriers, and treatment plans.
- Prepare comprehensive discharge summaries in compliance with regulatory and clinical requirements.
- Document all required information accurately in the electronic documentation system, maintaining patient records according to company policy.
- Record treatment minutes for services provided, adhering to all regulatory and clinical requirements.
- Participate in required meetings, including patient care conferences, utilization review meetings, and rehabilitation meetings.
- Engage in facility and company-required training, in-service, and conferences.
- Obtain necessary durable medical equipment (DME) or supplies in accordance with regulatory and practice requirements.
- Supervise and support Clinical Fellow Year (CFY) or student Speech-Language Pathologists.
- Adhere to ASHA and state-required practice acts, code of conduct, and ethics during professional practice.
- Maintain active ASHA certification and state licensure.
