William Penn Nursing and Rehab, a 121-bed skilled nursing facility located in the heart of Lewistown, Pennsylvania, is seeking a part-time Physical Therapist (PT) to join our team. As a Physical Therapist at our facility, you will have the opportunity to work with a diverse patient population, providing compassionate and high-quality care in a supportive environment.

About the Role:

As a Physical Therapist at William Penn Nursing and Rehab, you will be responsible for:

• Evaluating and treating patients with a range of physical disabilities and conditions

• Developing and implementing individualized treatment plans to achieve patient goals

• Providing education and support to patients and their families

• Collaborating with other healthcare professionals to ensure seamless care

• Maintaining accurate records and reporting changes in patient condition

Responsibilities:

• Conduct initial and ongoing assessments of patients' physical function and mobility

• Develop and implement treatment plans to address patients' physical needs

• Provide hands-on therapy interventions, including but not limited to exercise, stretching, strengthening, and gait training

• Monitor patients' progress and adjust treatment plans as needed

• Communicate effectively with patients, families, and healthcare teams to ensure coordinated care
