At Peace Healthcare Agency is seeking an experienced and dedicated DDD Coordinator to join our growing New Jersey Developmental Disabilities Division.
This position is ideal for a professional who understands the New Jersey Division of Developmental Disabilities (NJ DDD) system and is experienced in coordinating services, supporting individuals with developmental disabilities, maintaining accurate documentation, and working collaboratively with families, Support Coordinators, providers, and community partners.

The DDD Coordinator is responsible for coordinating and monitoring services for individuals with developmental disabilities while ensuring that services are delivered safely, effectively, and in accordance with NJ DDD requirements and person-centered service plans.
The ideal candidate will be highly organized, compassionate, detail-oriented, and comfortable managing multiple individuals, service providers, documentation requirements, and deadlines.
You will serve as an important link between the individuals we support, their families and guardians, Support Coordinators, Direct Support Professionals, healthcare providers, and community resources.
KEY RESPONSIBILITIESDDD Service CoordinationCoordinate services and supports for individuals receiving NJ DDD services.
Monitor service delivery to ensure supports are consistent with individual needs and approved service plans.
Maintain regular communication with individuals served, families, guardians, Support Coordinators, and service providers.
Assist with identifying appropriate community resources and support services.
Advocate for individuals and help ensure their preferences, goals, rights, and needs remain at the center of service delivery.
Monitor changes in individual needs and communicate concerns to appropriate team members.
Support the development and implementation of individualized, person-centered service plans.
Ensure services are aligned with individual goals, preferences, abilities, and desired outcomes.
Participate in planning meetings and collaborate with interdisciplinary teams.
Monitor progress toward established goals and recommend adjustments when appropriate.
Promote independence, community inclusion, dignity, and self-determination.
Maintain accurate, timely, and complete client records and service documentation.
Ensure documentation meets NJ DDD, Medicaid, state, federal, and agency requirements.
Monitor required assessments, service plans, progress notes, reports, and other documentation.
Utilize NJ DDD iRecord and electronic documentation systems as required.
Identify documentation or compliance concerns and take appropriate corrective action.
Assist with audits, quality assurance reviews, and regulatory monitoring.
Develop professional and trusting relationships with individuals served and their families or guardians.
Respond to questions, concerns, and service-related issues in a timely and professional manner.
Advocate for quality services and positive outcomes.
Help individuals access healthcare, community resources, recreational opportunities, employment supports, and other appropriate services.
Ensure concerns involving health, safety, rights, or quality of care are promptly communicated and addressed.
Communicate effectively with DSPs, program staff, Support Coordinators, healthcare professionals, and external providers.
Monitor service providers to ensure services are delivered according to established plans and requirements.
Address service delivery concerns and escalate significant issues appropriately.
Participate in team meetings and interdisciplinary discussions.
Help promote consistency and continuity of care across service settings.
Recognize and appropriately report incidents, concerns, and changes in client condition or circumstances.
Assist with incident investigations and corrective action when required.
Monitor service quality and identify opportunities for improvement.
Follow agency policies and NJ DDD reporting requirements.
Support ongoing quality improvement initiatives.
Bachelor's degree in Social Work, Human Services, Psychology, Healthcare Administration, or a related field.
Experience working with individuals with developmental disabilities, intellectual disabilities, behavioral health needs, or other vulnerable populations.
Working knowledge of New Jersey DDD services, regulations, and community-based programs.
Strong understanding of person-centered planning and service coordination.
Excellent written and verbal communication skills.
Strong organizational and time-management abilities.
Ability to manage multiple clients, responsibilities, and deadlines.
Strong documentation and record-keeping skills.
Ability to work independently while collaborating effectively with multidisciplinary teams.
Commitment to protecting client dignity, rights, confidentiality, and safety.
Previous experience as a DDD Coordinator, Support Coordinator, Case Manager, Service Coordinator, Program Coordinator, or similar role.
Direct experience working with NJ DDD.
Experience using NJ DDD iRecord.
Knowledge of Medicaid waiver services and community-based support programs.
Experience working with electronic health records or service documentation systems.
Experience with incident reporting, quality assurance, audits, and regulatory compliance.
Knowledge of positive behavior supports and individualized service planning.
Experience working with families, guardians, Support Coordinators, and multidisciplinary teams.
