Job Summary

Prestige Care Center of Fairfield is seeking a compassionate, skilled, and dedicated Physical Therapist (PT) to join our rehabilitation team on a per diem basis. The Physical Therapist will provide high-quality, patient-centered care to individuals working to restore mobility, strength, balance, function, and independence.

About the Role

As a Physical Therapist, you will evaluate patients, develop individualized treatment plans, and provide therapeutic interventions designed to maximize functional outcomes and quality of life.

Essential Responsibilities

• Evaluate patients' physical and functional abilities and identify individualized rehabilitation needs.

• Develop, implement, and modify individualized treatment plans based on patient goals, clinical findings, and response to treatment.

• Provide skilled physical therapy services, including therapeutic exercises, functional mobility training, gait training, balance activities, transfer training, and other appropriate interventions.

• Monitor patient progress and adjust treatment approaches as clinically appropriate.

• Educate patients, families, and caregivers regarding treatment programs, home exercises, mobility strategies, fall prevention, and safety precautions.

• Communicate effectively with patients, families, caregivers, physicians, nursing staff, and other members of the interdisciplinary team.

• Collaborate with Occupational Therapy, Speech Therapy, Nursing, and other healthcare professionals to support comprehensive, patient-centered care.

• Maintain accurate, timely, and complete documentation of evaluations, treatment sessions, progress, and discharge planning in accordance with facility and regulatory requirements.

• Participate in care planning and interdisciplinary meetings as appropriate.

• Promote patient safety and follow all facility policies, procedures, infection-control practices, and applicable state and federal regulations.

• Stay current with evidence-based practices, clinical guidelines, and developments in physical therapy to support high-quality patient outcomes.

• Maintain professional standards and protect patient privacy and confidentiality.

Qualifications

• Graduate of an accredited Physical Therapy program.

• Current and unrestricted Physical Therapist license in the applicable state.

• Current CPR certification, as required by facility policy.

• Previous experience in skilled nursing, rehabilitation, long-term care, or geriatric care is preferred but not required.

• Strong clinical, communication, organizational, and interpersonal skills.

• Ability to work independently while collaborating effectively with an interdisciplinary team.
