Physical Therapy Assistant

Job Summary:

Worland Health and Rehabilitation is seeking a Physical Therapy Assistant to join our exciting and fast paced team! Our housekeepers provide daily housekeeping services to maintain the Center's safety, sanitation, and attractive condition.

Essential Functions:

  • Comply with State practice act and/or American Physical Therapy Association (APTA) guidelines.

  • 2. Provide direct care to residents on physical therapy caseload.

  • 3. Implement treatment plans set-up by Physical Therapist.

  • 4. Provide clear, concise documentation in medical record and on appropriate forms as it relates to reimbursement guidelines and Center policy and procedure.

  • 5. Participate in continuous quality improvement activities.

  • 6. Identify and advise Rehabilitation Manager (RM) of any Center, staff, or operations issues that need to be addressed.

  • 7. Review caseload with supervising PT on a regular basis as established by best practice and/or State guidelines.

  • 8. Document supervision activities in each resident’s chart.

  • 9. Participate in discharge planning as it relates to caseload and PT prescribed programs.

  • 10. Participate in RAFT (Rehab Advocating Function Today) activities to identify residents in need of rehab services.

  • 11. Maintain productivity at level determined by the RM.

  • 12. Self-monitor productivity level daily.

  • 13. Participate in resident/family meetings as well as Center meetings when appropriate and as requested by the RM.

  • 14. Follow administrative policies and procedures related to emergency procedures, infection control, and safety.

  • 15. Maintain consistent and predictable hours of service in buildings as assigned.

  • 16. Document and sign each treatment encounter in medical record.

  • 17. Participate in writing progress notes as directed by Registered Physical Therapist (RPT) or RM.

Qualifications:

  • Associate degree from an accredited Physical Therapy Assistant program.

  • Current licensure in State of practice where applicable.
