About the Role:
Star Light Home Health is looking for a passionate and dedicated Physical Therapist (PT) to join our growing Medicare Certified Home Health team. Responsibilities:

• Conduct comprehensive physical therapy evaluations and develop individualized treatment plans for home health patients
• Deliver skilled PT services including therapeutic exercise, gait training, balance and fall prevention, and pain management
• Monitor and document patient progress in compliance with Medicare home health guidelines and agency policies
• Collaborate with the interdisciplinary care team including nurses, occupational therapists, and physicians
• Educate patients and caregivers on home exercise programs, safety techniques, and functional independence strategies
• Complete OASIS assessments and maintain accurate, timely clinical documentation
• Participate in care coordination and discharge planning to ensure continuity of care

Requirements:

• Active Colorado Physical Therapist (PT) license in good standing
• Graduate of an accredited Physical Therapy program (DPT or equivalent)
• Experience in home health, geriatrics, or Medicare-certified settings preferred
• Familiarity with OASIS documentation and Medicare home health regulations
• Valid driver's license, reliable transportation, and ability to travel to patient homes in the Grand Junction area
• Strong communication, time management, and critical thinking skills
• Compassionate, self-motivated, and comfortable working independently in the field
