Burlington Health & Rehabilitation Center is seeking a compassionate and dedicated Speech Therapist (Speech-Language Pathologist) to join our therapy team. This is an excellent opportunity to provide quality rehabilitation services in a supportive, collaborative environment focused on resident-centered care.

$7,500 Sign-On Bonus Now Available for Full-Time Employees!Responsibilities
  • Provide speech therapy services that promote excellence in rehabilitation care and quality patient outcomes.

  • Promote interdisciplinary cooperation and support the facility's geriatric rehabilitation program.

  • Evaluate patients in a timely manner according to state, payer, and facility requirements.

  • Screen patients upon admission and/or when requested by the Director.

  • Develop individualized treatment plans and appropriate goals based on patient evaluations.

  • Provide treatment according to the established plan of care.

  • Communicate and collaborate with other disciplines and staff as indicated by the treatment plan.

  • Provide routine documentation in accordance with Medicare, facility, and other payer guidelines.

  • Participate in family conferences when requested.

  • Record and submit all required documentation and treatment charges in a timely manner according to facility policies and procedures.

Qualifications
  • Current Vermont State License as a Speech-Language Pathologist.

  • Ability to work effectively as part of a dynamic and cooperative interdisciplinary team.

  • Strong assessment and evaluation skills.

  • Ability to develop individualized treatment plans and appropriate discharge plans.

  • Ability to participate in consultations, teaching, and meetings with residents, families, staff, and other healthcare professionals.

  • Strong communication and interpersonal skills.

  • Ability to educate and counsel patients, families, and staff as appropriate.
