Job Description
Woburn Rehabilitation and Nursing Center is seeking a highly skilled and compassionate Physical Therapist (PT) to join our veteran therapy team.
As a Physical Therapist, you will provide high-quality rehabilitation services designed to improve patients' mobility, independence, and quality of life. You will work collaboratively with an interdisciplinary team in a dynamic and supportive environment focused on clinical excellence and patient-centered care.
ResponsibilitiesEvaluate patients in a timely manner according to state, payer, and facility requirements.
Provide comprehensive physical therapy services, including evaluation, treatment planning, and implementation.
Develop individualized treatment plans and goals based on each patient's needs.
Communicate and collaborate with other disciplines and facility staff as indicated by the treatment plan.
Provide routine documentation in accordance with Medicare, facility, and other payer guidelines.
Participate in family conferences when requested.
Record and submit all required documentation and treatment charges accurately and in a timely manner.
Participate in consultations, education, and interdisciplinary meetings as needed.
Support appropriate discharge planning and patient transitions.
Promote patient safety, independence, dignity, and quality of life throughout the rehabilitation process.
Current Massachusetts state license as a Physical Therapist.
Strong evaluation, assessment, and clinical reasoning skills.
Ability to develop individualized treatment plans and rehabilitation goals.
Ability to work effectively as part of a dynamic and cooperative interdisciplinary team.
Flexibility to distribute workload according to departmental needs and individual responsibilities.
Ability to participate in consultations, teaching, and meetings with residents, staff, families, and other healthcare professionals.
Strong communication and interpersonal skills.
Ability to educate and counsel patients, families, and staff regarding the rehabilitation process.
