About the Role:
ANGELCARE HOME HEALTH SERVICES LLC is looking for a compassionate and skilled Physical Therapist to join our dedicated team in Vallejo, CA. This is a rewarding opportunity to make a meaningful difference in patients' lives by delivering high-quality, personalized care right in the comfort of their homes.

Responsibilities:

• Evaluate patients' physical conditions, functional limitations, and rehabilitation needs through comprehensive assessments
• Develop and implement individualized physical therapy treatment plans to restore mobility and improve quality of life
• Provide therapeutic exercises, manual therapy, and other evidence-based PT interventions in a home health setting
• Monitor and document patient progress, adjusting treatment plans as clinically appropriate
• Educate patients and caregivers on home exercise programs, safety techniques, and injury prevention
• Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated patient care
• Maintain accurate and timely clinical documentation in compliance with Medicare and regulatory standards

Requirements:

• Valid California Physical Therapist (PT) license in good standing
• Graduate of an accredited Physical Therapy program (DPT or equivalent)
• Home health or outpatient rehabilitation experience preferred
• Strong knowledge of Medicare home health guidelines and documentation requirements
• Excellent communication, interpersonal, and patient education skills
• Reliable transportation and valid driver's license for home visit travel
• CPR/BLS certification required or ability to obtain prior to start
