Job Responsibilities:

• Conduct comprehensive physical therapy evaluations and create personalized treatment plans.
• Provide hands-on physical therapy and exercise programs to patients of all ages.
• Educate patients and their families about exercises and lifestyle changes to manage and prevent injuries.
• Document patient progress and treatment plans in compliance with state regulations.
• Collaborate with other healthcare professionals to provide the best care possible.
• Maintain accurate and up-to-date records of patient treatments.

Qualifications:

• Licensed Physical Therapist (PT) in the state of New York.
• Doctor of Physical Therapy (DPT) Degree from an accredited institution.
• Current CPR Certification.
• Ability to work independently and as part of a team.
• Excellent communication and interpersonal skills.
• Previous experience in outpatient care or rehabilitation is preferred, but not required.

Required Licenses and Documents for New York Physical Therapists:

• Physical Therapy License issued by the New York State Education Department (NYSED).
• Proof of passing the National Physical Therapy Exam (NPTE).
• Proof of Continuing Education (as required by NYSED).
• CPR Certification (Adult/Infant/Child).

Salary: $120 for initial evaluation  - $90 for follow-up visits
