Full-Time Occupational Therapist (OT) – Skilled Nursing Facility
Lilac Manor | Rochester, NY
Position: Full-Time
Compensation: $45–$50 per hour
Lilac Manor is seeking a dedicated Full-Time Occupational Therapist (OT) to join our rehabilitation team. The Occupational Therapist will provide individualized, patient-centered care to help residents improve functional independence and achieve their highest level of ability in a supportive skilled nursing environment.
Responsibilities
Evaluate residents and develop individualized occupational therapy plans of care based on clinical and functional needs.
Provide skilled occupational therapy services to improve mobility, strength, coordination, and independence with activities of daily living.
Establish appropriate short-term and long-term treatment goals and monitor resident progress.
Complete timely and accurate evaluations, progress notes, treatment documentation, and discharge summaries.
Educate residents, families, caregivers, and facility staff on therapeutic techniques and recommendations.
Collaborate with physicians, nursing, rehabilitation staff, and other members of the interdisciplinary care team.
Recommend and train residents in the use of adaptive equipment when appropriate.
Ensure services and documentation meet facility standards and applicable state and federal requirements.
Qualifications
Valid New York State Occupational Therapist license.
Graduate of an accredited Occupational Therapy program.
Skilled nursing or long-term care experience preferred.
Experience with Rehab Optima preferred.
Strong clinical, communication, organizational, and documentation skills.
