About the Facility

Cordova Wellness & Rehab Center is a 242-bed skilled nursing facility offering:

  • Short-term rehabilitation

  • Long-term care

  • Outpatient therapy services

Located in the East Memphis/Cordova area, this in-house rehab team is known for strong leadership, a supportive culture, and a commitment to excellent clinical outcomes.

⚙️ Responsibilities

  • Provide skilled occupational therapy treatments in accordance with the supervising Occupational Therapist’s established plan of care.

  • Assist the OT with evaluations, tests, and complex therapeutic procedures.

  • Observe, document, and report resident conditions, responses, and progress.

  • Maintain accurate daily treatment documentation and progress notes.

  • Assist in consultations with families, physicians, and interdisciplinary team members.

  • Support the OT department's care, cleanliness, and organization, and ensure the proper care and maintenance of equipment.

  • Participate in in-service training, staff meetings, and continuing education.

  • Contribute to care conferences and quality improvement initiatives.

  • Follow all Ahava Therapy Services policies, regulatory requirements, and professional standards.

  • Understand payer guidelines and allocate therapy resources based on patient needs and discharge goals.

  • Demonstrate knowledge of OBRA regulations, HIPAA, LCD policies, and third-party reimbursement.

  • Maintain expected productivity levels while demonstrating strong teamwork, communication, and clinical judgment.

✔️ Qualifications

  • Graduate of an accredited Occupational Therapy Assistant program.

  • Active Tennessee OTA license (or eligibility).

  • Current BLS/CPR certification (or obtained during orientation).

  • Strong written, verbal, and observational skills.

  • Ability to work collaboratively within an interdisciplinary team.
