Cordova Wellness & Rehab Center is a 242-bed skilled nursing facility offering:
Short-term rehabilitation
Long-term care
Outpatient therapy services
Located in the East Memphis/Cordova area, this in-house rehab team is known for strong leadership, a supportive culture, and a commitment to excellent clinical outcomes.
⚙️ Responsibilities
Provide skilled occupational therapy treatments in accordance with the supervising Occupational Therapist’s established plan of care.
Assist the OT with evaluations, tests, and complex therapeutic procedures.
Observe, document, and report resident conditions, responses, and progress.
Maintain accurate daily treatment documentation and progress notes.
Assist in consultations with families, physicians, and interdisciplinary team members.
Support the OT department's care, cleanliness, and organization, and ensure the proper care and maintenance of equipment.
Participate in in-service training, staff meetings, and continuing education.
Contribute to care conferences and quality improvement initiatives.
Follow all Ahava Therapy Services policies, regulatory requirements, and professional standards.
Understand payer guidelines and allocate therapy resources based on patient needs and discharge goals.
Demonstrate knowledge of OBRA regulations, HIPAA, LCD policies, and third-party reimbursement.
Maintain expected productivity levels while demonstrating strong teamwork, communication, and clinical judgment.
✔️ Qualifications
Graduate of an accredited Occupational Therapy Assistant program.
Active Tennessee OTA license (or eligibility).
Current BLS/CPR certification (or obtained during orientation).
Strong written, verbal, and observational skills.
Ability to work collaboratively within an interdisciplinary team.
