Job Description
Homeside Rehab is seeking a compassionate and skilled Physical Therapist (PT) or Physical Therapist Assistant (PTA) to join our home health team in Oswego County, NY 13135.
As a valued member of our team, you will provide individualized, evidence-based therapy to patients in their homes while enjoying schedule flexibility and support from our clinical and administrative team.
Job ResponsibilitiesEvaluate patients and develop individualized treatment plans based on clinical assessment and patient goals
Provide evidence-based physical therapy interventions to improve mobility, strength, balance, functional independence, and overall quality of life
Maintain accurate and timely documentation of all patient care in accordance with regulatory requirements
Collaborate with the clinical team to ensure coordinated and effective treatment
Communicate clearly with office staff, patients, and caregivers regarding progress and care plans
Active New York State Physical Therapy license appropriate to discipline
Basic computer proficiency for electronic documentation
Strong clinical assessment and interpersonal communication skills
Ability to work independently in a home health environment
Valid driver's license and reliable transportation for home visits
Minimum 1 year of experience in Home Health, Outpatient, or Skilled Nursing Facility (SNF) settings preferred

Salary: $45–$80 per hour
