Responsibilities:

• Conduct thorough evaluations and create personalized treatment plans to promote optimal physical function and well-being

• Implement and modify treatment plans as needed to ensure effective rehabilitation outcomes

• Collaborate with interdisciplinary teams to ensure comprehensive care and communication with residents, families, and other healthcare professionals

• Provide education and guidance to residents and their families on proper body mechanics, safety, and wellness techniques

• Develop and maintain accurate and complete records of patient treatment and progress

• Participate in quality improvement initiatives to ensure the highest level of care and patient satisfaction
