Position OverviewWe are seeking a compassionate, dependable, and skilled Licensed Vocational Nurse (LVN/LPN)to join our healthcare team on a full-time basis.The LVN/LPN provides direct nursing care to residents under the direction and supervision established by the facility, RN, physician, and applicable regulations. This role is essential in supporting residents’ health, safety, comfort, and overall quality of care.Key Responsibilities
- Provide direct nursing care to residents in accordance with their care plans, physician orders, and facility policies.
- Administer routine medications and treatments within the LVN/LPN’s scope of practice.
- Monitor residents for changes in condition and promptly communicate concerns to the appropriate RN, physician, or healthcare provider.
- Perform nursing treatments and procedures according to established orders and protocols.
- Obtain and monitor vital signs and other relevant clinical information.
- Document medications, treatments, assessments, observations, and resident care accurately and timely.
- Assist in implementing and updating resident care plans as directed by the appropriate nursing team.
- Communicate effectively with residents, families, physicians, RNs, CNAs, and other members of the interdisciplinary healthcare team.
- Provide education and support to residents and families within the LVN/LPN scope of practice.
- Follow infection prevention, medication safety, and facility protocols.
- Respond appropriately to changes in residents’ conditions and escalate concerns when necessary.
- Maintain resident dignity, privacy, confidentiality, and safety at all times.
- Current and active LVN/LPN license in the state of practice.
- Graduate of an approved Practical/Vocational Nursing Program.
- Current CPR/BLS certification.
- Knowledge and experience with medication administration and nursing treatments preferred.
- Previous SNF, long-term care, or post-acute care experience preferred.
- Strong communication, organization, and clinical documentation skills.
- Ability to work effectively as part of an interdisciplinary healthcare team.
