Responsibilities
- Perform initial and ongoing assessments of residents’ mobility, balance, strength, gait, and functional limitations.
- Develop and implement person-centered treatment plans to restore or improve physical function, reduce pain, and prevent disability.
- Provide direct therapy interventions, including therapeutic exercises, gait training, balance retraining, and use of appropriate modalities.
- Monitor resident progress and adjust care plans to achieve therapy goals; participate in interdisciplinary care team meetings and discharge planning.
- Educate residents, families, and caregivers on home exercise programs and safety strategies to promote independence.
- Supervise and guide Physical Therapist Assistants (PTAs) and therapy aides in accordance with licensure requirements; document sessions accurately and timely; ensure compliance with regulatory and facility standards; support quality improvement initiatives.

Requirements
- Current Physical Therapist license in the state of practice (Missouri)
- Graduate of an accredited Physical Therapy program
- Experience in SNF or geriatric rehabilitation preferred
- Strong clinical, communication, and organizational skills
- Ability to work independently and as part of an interdisciplinary team
