About the Role

The Physical Therapist (PT) is responsible for evaluating residents, developing individualized treatment plans, and providing physical therapy services in accordance with established standards of practice, physician orders, company policies, and applicable regulations.

The Physical Therapist will work closely with the interdisciplinary team to help residents improve mobility, strength, balance, function, and independence while providing safe, compassionate, resident-centered care.

Essential Responsibilities

• Evaluate residents to determine their physical therapy needs and establish appropriate treatment plans. 
• Conduct initial, ongoing, and specialized evaluations as indicated. 
• Develop individualized treatment plans with short- and long-term goals. 
• Provide physical therapy treatments in accordance with the established plan of care and physician orders. 
• Monitor each resident's response to treatment and modify interventions as appropriate to achieve established goals. 
• Develop recommendations, strategies, and programs to maintain and improve resident function within their environment. 
• Establish individualized therapy schedules in coordination with the interdisciplinary team. 
• Communicate evaluation findings, treatment plans, progress, and recommendations to residents, families/responsible parties, and members of the interdisciplinary team. 
• Provide education to residents, families, caregivers, and facility staff regarding physical therapy procedures, strategies, recommendations, and programs. 
• Recommend and provide training on adaptive equipment and other devices as appropriate. 
• Participate in interdisciplinary team meetings and coordinate the resident's overall rehabilitation program. 
• Participate in discharge planning and assist with appropriate transition recommendations. 
• Supervise and direct Physical Therapist Assistants in accordance with applicable state practice guidelines. 
• Supervise Rehabilitation Aides, Physical Therapy Aides, students, and volunteers as directed. 
• Provide education to rehabilitation staff, facility employees, and the community as requested. 
• Maintain accurate and timely documentation in accordance with company procedures and applicable regulatory requirements. 
• Follow all requirements related to documentation and billing of physical therapy services. 
• Participate in Quality Assurance programs and performance improvement activities as required. 
• Maintain current knowledge of laws, regulations, and standards related to rehabilitation services. 
• Maintain compliance with HIPAA, confidentiality requirements, resident rights, infection control, universal precautions, OSHA standards, and company policies. 
• Maintain a safe environment and use therapy equipment appropriately and safely. 
• Report hazardous conditions, incidents, injuries, equipment concerns, or other safety issues immediately. 
• Respond appropriately to emergency and disaster situations. 
• Maintain professional and respectful communication with residents, families, coworkers, and interdisciplinary team members. 
• Attend required meetings, training, continuing education, and professional development activities. 
• Maintain professional appearance, hygiene, and dress standards. 
• Maintain reliable and predictable attendance. 
• Perform other duties as assigned. 

Qualifications

• Minimum of a Bachelor of Science degree in Physical Therapy from an accredited program. 
• Successful completion of the National Physical Therapy Examination (NPTE). 
• Current Physical Therapist license in the state(s) in which services are provided. 
• Current certification/registration as required by applicable state and regulatory requirements. 
• Previous experience in skilled nursing, long-term care, rehabilitation, or geriatric care preferred. 
• Strong clinical judgment and problem-solving skills. 
• Excellent communication and interpersonal skills. 
• Ability to work effectively as part of an interdisciplinary team. 
• Commitment to ongoing continuing education and professional development. 
• Ability to work with residents with varying physical, cognitive, and functional abilities. 

Physical Requirements

• Ability to walk throughout the facility and therapy department. 
• Ability to stand for extended and repetitive periods. 
• Ability to sit and work at a computer for extended periods. 
• Ability to bend, stoop, squat, kneel, reach, and navigate stairs as required to provide resident care. 
• Ability to safely assist residents with mobility, transfers, therapeutic activities, and positioning. 
• Ability to lift up to 50 pounds. 
• Ability to operate computers, telephones, and other office and therapy equipment. 
• Ability to travel between locations as needed. 
• Ability to maintain predictable attendance. 

Work Environment

This position is performed in healthcare and rehabilitation environments with frequent interaction with residents, families, nursing staff, physicians, therapists, and other interdisciplinary team members. The Physical Therapist may work throughout resident rooms, therapy areas, hallways, common areas, and other facility locations.

The position may involve exposure to infectious diseases, bodily fluids, medical equipment, cleaning products, and other conditions commonly found in healthcare settings. Employees are expected to follow all infection control, safety, OSHA, and facility requirements.
