Job Title: Certified Activity Director (LTC/SNF)

Avir at Camp Wood is seeking a compassionate and experienced Certified Activity Director (LTC/SNF) to join our dynamic team. Responsibilities:
Plan, organize, and implement a variety of recreational activities, including arts and crafts, games, and entertainment programs, in a Long-Term Care (LTC) and Skilled Nursing Facility (SNF) setting
Develop and maintain relationships with residents, families, and staff to ensure a positive and inclusive environment
Collaborate with interdisciplinary teams to create individualized activity plans that meet the unique needs and abilities of each resident
Conduct pre-activity assessments to identify and accommodate resident needs and limitations
Monitor and evaluate the effectiveness of activities and make adjustments as necessary
Maintain accurate records of resident participation and activity schedules
Participate in resident care conferences and contribute to the development of individualized care plans
Requirements:
Current certification as a Certified Activity Director (CAD) in the state of Texas
Bachelor's degree in Recreation, Therapeutic Recreation, or a related field

Strong communication and interpersonal skills
