Occupational Therapist (OT) / Occupational Therapy Assistant (OTA)

Homeside Rehab is seeking compassionate and motivated Occupational Therapists (OTs) and Occupational Therapy Assistants (OTAs) to provide one-on-one home-based therapy services in Sundown, NY 12740.

Job Responsibilities

• Provide skilled Occupational Therapy services in the patient's home

• Evaluate patients and develop individualized plans of care, as applicable to your professional scope of practice

• Carry out established treatment plans and therapeutic interventions

• Provide functional activities, therapeutic exercises, ADL training, strengthening, coordination activities, and other interventions within your professional scope

• Monitor patient progress and communicate changes or concerns to the appropriate supervising therapist and clinical team

• Maintain accurate and timely clinical documentation in accordance with company and regulatory requirements

• Communicate professionally with patients, caregivers, therapists, and office staff

• Provide one-on-one therapy services in the patient's home

Job Qualifications

• Active New York State Occupational Therapist (OT) or Occupational Therapy Assistant (OTA) license/certification, as applicable

• Basic computer proficiency for electronic documentation

• Strong interpersonal and communication skills

• Ability to work independently in a home-based setting

• Valid driver's license and reliable transportation for home visits

• Home health, outpatient, rehabilitation, or skilled nursing experience preferred

• Minimum of 1 year of clinical experience preferred

Job Type: Full-Time, Part-Time, Per Diem
Location: Sundown, NY 12740
Pay: $45–$70 per hour, based on discipline, experience, and case assignment
Schedule: Flexible – choose your availability
Shifts: Day and evening availability, Monday through Friday, with optional weekend availability
