Duties and Responsibilities:

Processing Home Health Referrals and checking eligibilities.
Data entry in the EMR and Requesting insurance authorization if needed.
Coordinating a team of healthcare professionals to deliver home health services.
Maintaining a relationship with patients and their families, and keeping them informed about the patient’s condition and care.
Scheduling patient appointments and ensuring regular follow-ups.
Communicating with doctors, clinicians and other healthcare providers about patient progress or required assistance with the patient.
Ensuring compliance with healthcare laws, regulations, and standards.
Monitoring and ordering medical supplies and equipment as needed.
Resolving any issues or concerns related to patient care.
Pay is negotiable.
