Responsibilities:

• Coordinate and manage home care services for clients, ensuring seamless care delivery
• Serve as the primary point of contact for clients, families, and caregivers
• Conduct client intake assessments and develop personalized care plans
• Schedule and match caregivers to clients based on needs, availability, and compatibility
• Monitor client satisfaction and proactively address concerns or changes in care needs
• Maintain accurate client and caregiver records in compliance with home health care regulations
• Collaborate with healthcare professionals and referral sources to support continuity of care

Requirements:

• Prior experience in home health care, case management, or a related healthcare coordination role
• Strong interpersonal and communication skills with a client-first mindset
• Excellent organizational and multitasking abilities in a fast-paced environment
• Proficiency with scheduling software and basic computer applications
• Knowledge of home care regulations and compliance standards preferred
• Empathetic, dependable, and solutions-oriented team player
• High school diploma required; Associate's or Bachelor's degree in healthcare or related field a plus
