Ready for your next Home Health opportunity? Ardor Health Solutions is seeking a Travel Physical Therapist for a rewarding Home Health assignment based out of Gassaway, West Virginia! This is a great opportunity for a PT who enjoys one-on-one patient care, independence, and a consistent weekday schedule.
What You'll Do
- Perform comprehensive physical therapy evaluations in patients' homes.
- Develop individualized treatment plans based on physician orders and patient goals.
- Help patients improve mobility, strength, balance, safety, and functional independence.
- Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility.
- Collaborate with physicians and the interdisciplinary healthcare team.
- Complete accurate and timely clinical documentation.
Assignment Highlights

✅ Day Shift | 8:00 AM – 4:30 PM

Requirements
- West Virginia PT license can be pending — you do not need to have it in hand to get started!
- Home Health experience preferred.
- OASIS experience preferred.
- Current BLS certification.
- Valid driver's license and reliable transportation.
- Strong communication, organization, and independent clinical skills.
