Certified Occupational Therapy Assistant/OT - Caseload and $1500 Bonus

We have an immediate need for a passionate and outgoing COTA or OT (current provider is moving out of state), to help serve children and young adults with Intellectual and Developmental Disabilities (ID/DD), in a Home-Health setting.

This position is currently offering a caseload of 18 kiddos, located in the following zip codes:

85122 - 8 kiddos (including 1 set of siblings)

85123 - 7 kiddos (including 2 sets of siblings)

85194 - 2 kiddos

85193 - 1 kiddo

****Even if you're not able to takeover the entire caseload, but are interested in a few of them, please contact us!****

Compensation and Bonuses

• $90-$95 per visit/hour (This is COTA rate; Licensed OT rates would be higher)
• $1500 sign on bonus (you would qualify after 60 days of service with the agency)

There is also an opportunity to grow the size of your caseload, to a level you're comfortable with. We can tailor the search for additional kiddos to fit your desired location and schedule/availability.

Application requirements

• Arizona Fingerprint Clearance Card
• Must be currently licensed as a COTA or OT in the State of Arizona
• One year experience is preferred, but not required
