What You'll Do
As a Home Health Physical Therapist, you will:
- Perform comprehensive physical therapy evaluations and assessments
- Develop individualized treatment plans based on physician orders and patient needs
- Provide therapeutic exercises, gait training, balance and mobility interventions, and manual therapy as appropriate
- Evaluate patients' functional abilities and track progress throughout their episode of care
- Educate patients and caregivers on exercises, mobility, fall prevention, and safety strategies
- Communicate with physicians, nurses, and other members of the care team
- Complete accurate and timely clinical documentation
- Help patients achieve greater strength, mobility, independence, and confidence in their everyday lives
What We're Looking For
We're looking for a PT who is clinically confident, dependable, compassionate, and enjoys working independently.
Qualifications:
- Graduate of an accredited Physical Therapy program
- Active Physical Therapist license in the state(s) where you will be practicing
- Home Health experience preferred
- Current CPR/BLS certification
- Valid driver's license
- Reliable transportation and current auto insurance
- Current TB clearance
- Arizona Level One Fingerprint Clearance Card, if applicable
