Speech-Language Pathologist (SLP)

Valley Oaks Post Acute is seeking a full-time Speech-Language Pathologist (SLP) to join our in-house rehabilitation team.

This is an excellent opportunity for a Speech Pathologist who is passionate about helping residents improve communication, cognition, swallowing, and overall independence. As part of our collaborative in-house rehabilitation team, you will provide personalized, resident-centered therapy and develop individualized treatment plans that support meaningful progress, improved function, and a higher quality of life. Key Responsibilities

  • Evaluate and treat speech, language, cognitive, and swallowing disorders

  • Develop and implement individualized treatment plans

  • Educate residents, families, caregivers, and clinical staff

  • Collaborate on diagnostic swallowing procedures, including MBSS and FEES

  • Maintain accurate, timely, and compliant documentation

  • Communicate changes in resident conditions to the care team

  • Support restorative nursing and interdisciplinary care initiatives

  • Ensure compliance with all professional, legal, and confidentiality standards

Qualifications

  • Active Speech-Language Pathology state license in good standing

  • Current ASHA certification

  • License in good standing with all applicable boards and agencies

  • Skilled nursing or long-term care experience preferred

  • Knowledge of Net Health Electronic Documentation System preferred

  • Strong communication and clinical skills
