Amcare Pro Home Health, Palliative Care & Hospice is seeking a compassionate and dedicated Physical Therapist for a PRN field position providing exceptional in-home therapy services throughout the Fort Worth area.

Responsibilities

• Evaluate patients and provide skilled physical therapy services according to physician orders and individualized treatment plans
  
  
• Implement therapeutic exercises and interventions to improve mobility, strength, balance, pain control, and functional independence
  
  
• Assess patients’ home environments and recommend equipment and safety modifications to support successful recovery
  
  
• Educate patients and caregivers on home exercise programs, mobility techniques, and fall-prevention strategies
  
  
• Complete timely and accurate clinical documentation in the electronic medical record
  
  
• Collaborate with the interdisciplinary team to coordinate patient care and communicate patient progress
  
  
• Maintain compliance with agency policies, Medicare regulations, and all applicable state and federal standards
  
  
• Travel to patients throughout Fort Worth and the surrounding area
  
  

Qualifications

• Current Texas Physical Therapist license in good standing
  
  
• Graduate of an accredited physical therapy program
  
  
• Minimum of one year of physical therapy experience
  
  
• Home health experience preferred
  
  
• Current BLS certification
  
  
• Valid Texas driver’s license
  
  
• Reliable transportation and current automobile insurance
  
  
• Strong organizational, communication, and clinical skills
  
  
• Experience with electronic medical records preferred
