Job Overview

Deer Meadows Home Health is seeking a dedicated Occupational Therapist (OTR/L) for a Full-Time or PRN opportunity in Home Health and Community-Based settings, including Assisted Living (ALF), Independent Living (IL), and Personal Care.

This role offers the opportunity to provide one-on-one patient care across a diverse population while working with a collaborative interdisciplinary team focused on improving patient independence and quality of life.

Key Responsibilities

• Evaluate patients and develop individualized occupational therapy treatment plans

• Provide skilled interventions to improve ADLs, safety, and functional mobility

• Deliver therapy services in homes and community-based settings (ALF, IL, Personal Care)

• Educate patients and caregivers on adaptive equipment and home safety

• Collaborate with nurses, therapists, and interdisciplinary team members

• Complete timely documentation in Home Care Home Base (HCHB EMR)

• Independently manage caseload and daily schedule

Qualifications

• Graduate of an accredited Occupational Therapy program

• Active Pennsylvania OT license (OTR/L) or eligibility

• Home Health experience preferred

• Experience with Home Care Home Base (HCHB) a plus

• Strong clinical judgment, communication, and organizational skills

• Valid driver’s license and reliable transportation
