Senior Social Worker - Onsite

We are seeking an experienced and compassionate Senior Social Worker to join the Arizona State Veterans’ Home and provide high-quality social services to veterans and their families.

The selected candidate will oversee the Social Services program, supervise staff, maintain a personal caseload, complete resident assessments and care plans, and ensure compliance with federal, state, CMS, and facility requirements.

Key Responsibilities:

• Supervise and coordinate the daily activities of Social Services staff.
• Conduct resident assessments, counseling, and case management.
• Develop and monitor individualized care plans.
• Coordinate safe and clinically appropriate discharge planning.
• Ensure compliance with PASRR, MDS, Medicare, and Resident Rights requirements.
• Coordinate referrals and consults with the VA and community resources.
• Participate in interdisciplinary meetings, committees, training, and facility improvement initiatives.
• Maintain accurate and confidential resident documentation.

Required Qualifications:

• Master’s degree in Social Work (MSW) — Required
• Active Arizona Social Work License
• Minimum 1 year of supervisory experience in a healthcare setting
• Experience in long-term care preferred/required
• Knowledge of geriatric care, behavioral health, CMS regulations, and social service programs
• Strong communication, counseling, assessment, case management, and leadership skills
• Ability to obtain and maintain an AZ DPS Fingerprint Clearance Card
• Current negative TB/Mantoux test as required
