Home Health Field Supervisor Job Description
Job Title: Home Health Field Supervisor
Department: Operations / Clinical Services
Reports To: Administrator / Director of Operations
Position Summary
The Home Health Field Supervisor is responsible for overseeing the daily performance of Direct Support Workers (DSWs), Certified Nursing Assistants (CNAs), Personal Care Attendants (PCAs), and other field staff to ensure the delivery of high-quality, person-centered care in accordance with agency policies and applicable federal and state regulations. This position provides field supervision, staff training, quality assurance, and operational support while promoting positive patient outcomes and employee accountability.
Essential Duties and Responsibilities
Staff Supervision
- Supervise, coach, and evaluate home care staff to ensure quality service delivery.
- Conduct routine field supervisory visits and competency assessments.
- Provide mentoring, corrective coaching, and performance improvement plans when necessary.
- Participate in employee hiring, orientation, and onboarding.
- Assist with employee retention initiatives and staff recognition.
- Investigate employee concerns, complaints, and incidents.
Patient Care Oversight
- Conduct home visits to evaluate the quality and appropriateness of services.
- Monitor implementation of individualized care plans.
- Ensure participants receive authorized services according to their plans of care.
- Address patient and family concerns promptly and professionally.
- Perform follow-up visits after incidents, hospitalizations, or service interruptions.
- Promote person-centered care and patient rights.
Regulatory Compliance
- Review documentation for completeness, accuracy, and timeliness.
- Monitor EVV/LASRS documentation and verify service delivery.
Staff Training and Development
- Conduct orientation and ongoing education for field employees.
- Evaluate competencies and provide retraining as needed.
Scheduling and Operations
- Assist with staffing assignments to ensure adequate coverage.
- Monitor overtime and productivity.
- Respond to staffing emergencies and call-offs.
- Coordinate replacement staffing when necessary.
- Assist with scheduling to maximize utilization of authorized hours.
- Participate in the agency's on-call rotation.
Quality Assurance
- Conduct supervisory visits according to agency policy.
- Track employee evaluations and supervisory visit compliance.
Care Coordination
- Participate in care conferences and interdisciplinary meetings.
Administrative Responsibilities
- Maintain accurate personnel and participant records.
- Maintain confidentiality of all protected health information.
Qualifications
Education
- Associate's degree preferred.
Experience
- Minimum of two to three years of experience in home health, personal care services, healthcare, or human services.
- At least one year of supervisory or leadership experience preferred.
Knowledge and Skills
- Strong leadership and team-building skills.
- Excellent communication and interpersonal skills.
- Effective problem-solving and conflict-resolution abilities.
- Strong organizational and time-management skills.
- Proficiency with Microsoft Office and electronic health record/documentation systems.
- Ability to maintain confidentiality and professionalism.
Physical Requirements
- Ability to travel between participant homes and agency offices.
- Valid driver's license, reliable transportation, and proof of insurance.
- Ability to lift up to 25 pounds.
- Ability to sit, stand, walk, bend, kneel, and climb stairs during home visits.
- Ability to work in a variety of home environments.
Work Environment
- Combination of office work and field visits.
- Frequent travel within the service area (Region 9).
- Occasional evenings, weekends, and on-call responsibilities.
- Fast-paced environment requiring flexibility and sound judgment.
