Physical Therapist (PT) specializing in Home Health services is needed for a position based in a local area. The role involves providing outpatient care within the community, focusing on geriatric patient populations. The position offers a flexible work environment, requiring timely documentation and communication.
Job Details:
- Schedule: Day shift, 8 hours per day, minimum of 40 hours per week.
- Required Qualifications: Valid license in hand with active drivers license and personal transportation; minimum of 1 year of relevant experience; recent experience with home health settings; proficiency in HCHB (Homecare Homebase); BLS certification; ability to manage patient care for geriatric populations, including post-operative and chronic conditions.
- Preferred Qualifications: Experience in home health, familiarity with OASIS documentation for staff completing Start of Care assessments.
- Additional Information:
- No first-time travelers accepted for this role.
- Candidates must be flexible with their territory, which covers up to a 60-mile radius from the hub/office.
- Timely communication and documentation are mandatory.
- Orientation is scheduled on the first Monday.
- Each traveler must have own transportation; travel pairs are accepted but will be assigned to separate territories.
- Modules during orientation are billable and completed within the training period.
- On-call rotation may be required; details will be discussed during the interview.
- Travelers are expected to be flexible with territory and ensure timely documentation.
