Physical Therapist - Home Health

Key Responsibilities- Plan and conduct segments of treatment programs designed to restore and improve physical, social, and mental functions in alignment with department objectives
- Serve as co-leader in an interdisciplinary approach to patient care and patient groups
- Utilize HCHB and OASIS documentation platforms for patient records and clinical documentation
- Utilize EPIC for electronic health record management
- Provide home health physical therapy services to patients across the assigned service area, with mileage reimbursed at the government rate
- Doctor of Physical Therapy (DPT) degree required if license was obtained on or after January 2016, or foreign equivalent
- Active Physical Therapy licensure in the state of practice: Required
- AHA BLS or CPR certification (AHA-BLS or AHA-CPR) from an approved vendor: Required
- Valid Driver's License required; must be at least 21 years of age or older
- Minimum 1 year of home health experience required
- Proficiency in HCHB, OASIS, and EPIC required
- Experience working in an interdisciplinary or team-based care model
- Strong organizational and time management skills suited for an independent, field-based role
- Monday–Friday schedule with no weekends and no on-call requirements, offering a strong work-life balance
- Mileage reimbursed at the government rate, supporting field-based travel needs
