\n \nOpportunity:
\nSocial Worker - Social Worker
\nJob Details:
\n• Work day shifts from 8:00 AM to 4:30 PM with occasional weekend requirements
\n • 40 guaranteed hours per week
\n • 13-week contract
\n • May be low censused and placed on call; call back minimum is 2 hours
\n • Work across Med Surg and Critical Care Clusters in all inpatient units using a Layered Model in tandem with RN Team for discharge planning
\n • Patient ratio of 8 to 20 patients per day based on acuity
\n • Master's Degree in Social Work (MSW) preferred; Master's Degree required
\n • Minimum 1 year of experience in social services and counseling in a hospital acute care setting with a diverse population required
\n • Proficiency with Epic charting system required
\n • Wisconsin license required
\n • Holiday rotation as needed
\n • Business attire required
\n • Traveler must reside 50 miles outside of the facility
